Follow these handy shortcuts to download the required parent paperwork for 2024 Zoo Camp participants. This year there are 2 required forms:
- Zoo Emergency Contact and Pick Up Permissions Form
- Zoo Health Advisory and Accommodation Form
Once completed, upload these documents to your account in the CivicRec online registration portal, or email them (pdf preferred) to zooeducaiton@lafayette.in.gov. These forms are required at least 14 days prior to the start of your camp session in order to participate.
